Under 18 - Regular Class R Driver License

You must provide the following:

1. A completed Application with notorized signatures of both parents or guardians. If parents are divorced, parent with custody must sign.

2. A social security card or a printout from the Social Security Administration. (Note: metal social security cards are not acceptable.)

3. A certified copy of your birth certificate. (Note: Hospital certificates are not acceptable.)

4. You must show proof that you are in school by presenting a Certification Of School Attendance form obtained from the school that you are attending. (Note: This form is not valid if over 30 days old.) If applicant is under 18 years of age and married, a Certification of School Attendance form is not required. However, applicant my present a certified copy of the marriage license. Driver license will only be valid for one year or until next birthday.

You must pass the driving skills test.

You must pay the appropriate Fee - cash only.
